Client Intake Form: What do you need to know?

Can my client change/edit their Intake form after it’s been submitted? Once the client submits the form, it is locked. A client then contacts their provider to make further changes. This is done to prevent the client from adding something important after the midwife reviews the form, resulting in the midwife not ever seeing the added information. 

To unlock the form open the client's chart and select Forms from the left-hand Client Navigation menu. At the top of the page you will see the status of the form, and depending on the status of the form, options to lock, unlock, or to send a reminder to complete the form to the client by email.

How is the client’s view of the intake different than a Provider’s? The sections are the same, but the questions are asked in more common language (not overly medical).  An example of that is:

  • Client sees: Including this pregnancy, how many times have you been pregnant?
  • Provider sees: Gravida: 1.

How can I see what the client sees? You can create a test client to use for experimenting and training other members of your practice. This will allow you to experience Maternity Neighborhood from the client's perspective. If you let us know what the chart number is for your fake client, we can even exempt it from your yearly client count. If you wish, you can do this using the same email address as the one you use as a midwife and then switch to that profile using the menu drop-down under your name in the upper right-hand corner.

Where do I access the Client’s Intake form? The Client’s Intake form is found in the “Forms” tab in the left-hand Client Navigation menu. Selecting this tab takes you to the first page of the form, with an index on the right-hand side.  The default view is of all information entered by the client or by another user in your practice. There are additional fields available as well as a place to make comments in each section for additional information. Click “Edit” in the upper right-hand corner of each category to access the additional fields or comments sections.  

Using the Intake form: Practice users can flag any item on the Intake form to be included in the 'Alerts' box on the client’s Overview by clicking the gray flag that appears when you hover over an item on the form. The flag will appear red when that item is listed in Alerts. Click the flag again to remove it from the Alerts.  A practice user may also make assessment notes in each section of the Intake form - these assessment notes are NOT visible to the client and can be used to record reasons a client may not be suitable for care at your practice. You can also record additional details after speaking to the client about information provided on the form.
